Fresh vs Frozen Embryo Transfer: Which Is Right for You?

One of the most common questions patients ask once fertilization is confirmed is simple: should the embryo be transferred now, in this fresh cycle, or frozen and transferred later? A decade ago, fresh transfer was the default and freezing was a backup plan. Today, thanks to major advances in freezing technology, that balance has shifted — and for many patients, frozen embryo transfer (FET) is now the recommended path rather than the fallback. This article explains what each approach actually means, why frozen transfer has become so effective, and how the decision gets made for your specific case.

In a fresh embryo transfer, an embryo created during an IVF cycle is transferred into the uterus a few days after egg retrieval — typically on day 3 or day 5 of development — within the same cycle, without ever being frozen.
In a frozen embryo transfer, the embryo is cryopreserved shortly after fertilization (usually at the blastocyst stage, on day 5 or 6) and stored until a later cycle. When the transfer is scheduled, the embryo is thawed and placed into a uterus that has been separately prepared — either through a natural ovulatory cycle or with hormone medication — specifically for that transfer.
The core difference isn’t just timing. It’s that a fresh transfer happens in a uterine environment that has just been through ovarian stimulation, while a frozen transfer happens in a cycle designed and prepared purely for implantation.
How Vitrification Changed the Picture
Older, slow-freezing methods carried a real risk of ice crystal formation damaging embryo cells during freezing and thawing. That risk is why, for many years, fresh transfer was considered the safer and more reliable option by default.
Vitrification changed that. This ultra-rapid freezing technique cools embryos so quickly that ice crystals don’t have time to form — the cells are essentially locked in a glass-like state instead. Survival rates after thaw with vitrification are very high, and embryology data over the past decade has shown that vitrified embryos generally perform comparably to fresh ones after thaw. This is the single biggest reason frozen embryo transfer has gone from a backup option to a genuine first-choice strategy in many IVF programs, including ours.
Why Frozen Transfer Is Often Favoured
There are several clinical reasons a fertility team may recommend freezing all embryos and transferring later rather than proceeding fresh.
- Better endometrial receptivity. Ovarian stimulation raises estrogen and progesterone to levels far above a natural cycle, which can advance or otherwise alter the uterine lining in ways that aren’t always ideal for implantation. A frozen cycle allows the lining to be prepared separately and more precisely — either naturally or with a controlled hormone protocol — timed exactly to the embryo’s stage of development.
- Lower risk of OHSS. Patients who respond strongly to stimulation and produce many eggs are at higher risk of ovarian hyperstimulation syndrome (OHSS). Freezing all embryos and delaying transfer avoids adding a pregnancy on top of a hyperstimulated body, which is one of the most effective ways to prevent moderate-to-severe OHSS.
- Time for genetic testing (PGT). If preimplantation genetic testing is part of the plan, embryos must be biopsied and results awaited before a transfer decision can be made — this is only possible with frozen embryos, since testing takes days to weeks.
- Flexibility and reduced pressure. Freezing separates the stimulation and retrieval phase from the transfer phase entirely. This gives your body time to recover from stimulation medication and gives your clinical team room to choose the optimal cycle and lining conditions, rather than transferring on a fixed fresh-cycle timeline regardless of how the lining looks.
The “Freeze-All” Strategy
Because of the advantages above, many IVF programs now routinely recommend a freeze-all approach: every embryo from the stimulation cycle is vitrified, and the first transfer is planned as a frozen transfer in a subsequent, dedicated cycle. This isn’t a sign that anything went wrong with the retrieval — it’s a deliberate strategy used specifically because it tends to give the uterine lining and the embryo the best chance to align.
Freeze-all is particularly common when estrogen levels are very high after stimulation, when OHSS risk is elevated, when PGT is planned, or simply as a clinic’s standard protocol based on the patient’s individual profile.
When Fresh Transfer Is Still the Right Choice
Frozen transfer being favoured in many cases doesn’t mean fresh transfer is outdated or inferior across the board. Fresh transfer can still be the appropriate choice when:
- The uterine lining looks well developed and receptive on the day of retrieval, with no signs of premature hormonal shift.
- Estrogen and progesterone levels after stimulation are within a normal range, with low OHSS risk.
- No genetic testing is planned, so there’s no clinical reason to delay.
- The patient and clinical team prefer to avoid the extra time, medication, and travel logistics of a separate frozen cycle.
The decision is never made on preference alone — it’s based on how your individual cycle responds, your hormone levels, your lining, and your treatment plan.
Comparing Success Rates Honestly
Patients understandably want a direct answer to “is frozen embryo transfer better?” The honest answer is: it depends on the individual, and neither approach is guaranteed to succeed. What the broader body of fertility research does show is that, with modern vitrification, frozen transfer success rates are generally comparable to fresh transfer, and in cases where a fresh transfer would occur in a compromised hormonal environment, FET often performs better. Fresh transfer can be equally effective when the lining is well-timed and hormone levels are favourable.
There’s no universal winner between fresh and frozen. The right comparison isn’t “which is better in general” — it’s “which is better for this cycle, this patient, and this embryo.” That assessment is something your fertility specialist makes by reviewing your hormone levels, ultrasound findings, and overall treatment goals, not by applying a blanket rule.
What This Means for International Patients

- Fresh transfer means a single trip covering stimulation, retrieval, and transfer within roughly two to three weeks, which can simplify travel and time away from work.
- Frozen transfer typically means two separate trips — one for stimulation and retrieval, and a second, usually shorter one, for the transfer cycle once the embryo (and, if applicable, genetic testing results) are ready. Many patients find this easier to plan around, since the second trip is shorter and can be scheduled at a time that suits their personal and professional calendar.
- Vitrified embryos can be stored securely for an extended period, so there’s no urgency to schedule the frozen transfer immediately — this gives international patients meaningful flexibility in timing their return.
